What Is Medication Therapy Management — and How Can It Help You?

MTM is a one-on-one service with a clinical pharmacist designed to optimize your medications and improve your health outcomes.

If you're taking multiple medications — especially specialty drugs alongside other prescriptions, supplements, and over-the-counter products — it can be difficult to keep track of everything: dosing schedules, potential interactions, side effects, and whether each drug is still doing what it's supposed to do. Medication Therapy Management (MTM) is a structured clinical service designed to address all of this in one personalized consultation.

MTM sessions at Sorrento are conducted by our licensed clinical pharmacists and can be done by phone, video, or in person. Call (818) 572-9982 to schedule yours.

What happens during an MTM session?

A clinical pharmacist conducts a comprehensive review of all your medications — every prescription, OTC product, vitamin, and supplement you take. We look for drug interactions, duplicate therapies, medications that may no longer be necessary, dosing errors, and opportunities to simplify your regimen or reduce side effects. At the end of the session, you receive a personalized Medication Action Plan in writing — something you can share with all your doctors.

Common issues uncovered during MTM

An MTM session with a clinical pharmacist gives you a clear, written medication action plan.

In our experience, MTM sessions frequently uncover issues that had gone unnoticed: a supplement interacting with a blood thinner, a medication prescribed by two different specialists that effectively duplicates another, a drug being taken at the wrong time of day affecting its absorption, or a side effect that turns out to be caused by a medication rather than the underlying disease. Catching these issues early can prevent serious health consequences.

Who benefits most from MTM?

MTM is particularly valuable for patients managing multiple chronic conditions, those who see more than one specialist, anyone who has recently started a specialty medication, and caregivers managing medications for a family member. If you're ever confused about why you're taking a medication, whether it's still needed, or what to do if you miss a dose — an MTM session is the right starting point.

Is MTM covered by insurance?

Many Medicare Part D plans cover MTM services at no cost for eligible enrollees — typically patients with multiple chronic conditions who take multiple medications. Coverage under commercial insurance varies. Even when it isn't covered by insurance, the value of identifying dangerous interactions or eliminating unnecessary medications typically far outweighs the cost.
